Intradiscal Electrothermal Therapy (IDET) is a procedure in which a semirigid heating element is placed into the center of a painful disc and the disc is heated up to 90 degrees Celsius. This denatures the nerves inside the disc thus rendering the disc painless without damaging the larger nerves outside the disc. Percutaneous Laser Discectomy (PLDD) is a procedure by which a contained disc herniation is decompressed using a laser scope. Both procedures are purcutaneous and outpatient. Complications are very rare, there is essentially no "down time", and rehabilitation is rapid.
Spinal cord stimulation (SCS) is a method by which a percutaneous catheter with very small electrodes is implanted in the spinal column which blocks pain by applying a small electrical current to the spinal cord. It is powered by a pacemaker battery and is very eefective at the control of neck and upper extremity or back and lower extremity pain. A drug infusion pump (DAS) is a pump an implantable pump that is inserted under the abdominal skin and connects to the spine with a subcutaneous catheter. The pump is programmable and delivers medication to the spine at a continual or variable basis depending on the patient's needs. Both procedures are generally performed on an outpatient basis andd are very effective at managing difficult pain syndromes.
Radiofrquency nerve ablation is a method of safely taking the sensory nerve supply away from a painful structure. This is not unlike the procedure that a dentist does when he performs a "root canal" although the radiofrequency procedure is usually much less uncomfortable than a root canal. It is performed by placing an insulated probe immediately adjacent to the sensory nerve of a painful structure and applying a small microwave signal. It is a safe, outpatient procedure with a high success rate when properly applied.
